چکیده

A 36-year-old woman presented with vertical diplopia, nausea, and disequilibrium. Maddox rod testing was performed in the upright and supine positions (figures 1 and 2). Skew deviation is a vertical misalignment caused by a supranuclear lesion in the vertical vestibuloocular reflex pathways, including the vestibular nerve, brainstem, or cerebellum. It arises from perturbed utricular inputs, and the amplitude of the ocular deviation is therefore sensitive to gravitational forces. The upright-supine test helps localize the cause of vertical diplopia by distinguishing skew deviation from infranuclear causes. With skew deviation, the vertical deviation is substantially reduced when the patient is supine, whereas with infranuclear lesions it is not.

Teaching Video NeuroImages: positional exophthalmos in orbital varices.

A 56-year-old woman presented with intermittent vertical diplopia and exophthalmos of the left eye, which become apparent when she was bending over. Examination disclosed mild limitation of supraduction and lid lag of the left eye. Bending over induced profound exophthalmos of the left eye (video) and aggravated vertical diplopia. The exophthalmos was confirmed by an exophthalmometer, which sho...

Understanding skew deviation and a new clinical test to differentiate it from trochlear nerve palsy.

Skew deviation is a vertical strabismus caused by a supranuclear lesion in the posterior fossa. Because skew deviation may clinically mimic trochlear nerve palsy, it is sometimes difficult to differentiate the 2 conditions. In this review we compare the clinical presentations of skew deviation and trochlear nerve palsy and examine the pathophysiology that underlies skew deviation. We then descr...

Ability of an upright-supine test to differentiate skew deviation from other vertical strabismus causes.

OBJECTIVE To determine the sensitivity and specificity of a new upright-supine test to differentiate skew deviation from trochlear nerve palsy and other causes of vertical strabismus in a large number of patients. METHODS The study consisted of 125 consecutive patients who sought treatment from January 1, 2008, through December 31, 2010, for vertical strabismus of various causes: skew deviati...

Head position-dependent changes in ocular torsion and vertical misalignment in skew deviation.

OBJECTIVES To investigate whether ocular torsion and vertical misalignment differ in the upright vs supine position in skew deviation and to compare these findings with those in trochlear nerve palsy. METHODS Ten patients with skew deviation, 14 patients with unilateral peripheral trochlear nerve palsy, and 12 healthy subjects were prospectively recruited. With subjects first in the upright p...

Tongue resting pressure of the tongue anchorage pad in different body positions: a pilot study.

We designed a modified transpalatal arch (tongue anchorage pad, TAP) to help control the vertical dimension. This study aimed to evaluate its efficiency by measuring the tongue resting pressure at different anteroposterior positions of the TAP in the upright and supine positions and to investigate the effect of changes in body position. Our study recruited 17 volunteers with individual normal o...
